Organisation Details - for Adult Learning Disability Service Community Team

 

What They Do

 

Part of Adult Learning disability Service. This is the main support service for people with learning disabilities and their families. The team consists of social workers, community nurses, carers assessor, psychologists, speech and language therapist, psychiatrist, occupational therapist, physiotherapist and community support workers. The team is responsible for arranging and reviewing people’s needs against the Council’s eligibility criteria and for completing carer’s assessments. It is also responsible for arranging and funding from the Community Care budget to meet people’s needs, and for reviewing their care packages each year.
